The Low FODMAP Diet | Doctor Explains IBS, Bloating & Gas Treatment

In today’s episode of Dr Nora, I take you through the FODMAP diet which has been scientifically proven to help those suffering with IBS symptoms on the guidance of their medical doctor and dietician.

WHAT IS IBS?

IBS or Irritable Bowel Syndrome is a condition that affects the large bowel which can cause some uncomfortable symptoms. Such symptoms include, tummy pain, bloating, loose motions or flatulence. It can affect up to 3 in 10 Australians and can be caused by certain triggers, such as stress, caffeine or certain medications to name a few examples. It is important that this condition is diagnosed by your medical doctor as other conditions may also present in a similar way.

FODMAP

FODMAP stands for Fermented Oligosaccharides, Disaccharides, Monosaccharides and Polyols. It pertains to certain groups of sugars that are poorly absorbed in the intestine leading to flatulence, bloating and cramps if consumed. Often those with IBS will be advised to try and follow a low FODMAP diet to reduce the incidence of symptoms on the guidance of their doctor or dietician.

Low FODMAP foods include bananas, blueberries, red bell peppers, pumpkin, rice, oats, lactose free milk and tofu. Non-marinated meats are also considered low FODMAP.

High FODMAP foods include apples, mangoes, milk from cows, soft cheeses, broccoli, chickpeas and mushrooms.

Usually a diet including low FODMAPs will be advised for a period of 2-6 weeks, after which, some food groups can be re-introduced as a challenge for up to 3 days. If symptoms persist with the food group then that particular food is to be avoided. It is vital to seek the guidance of a medical practitioner, as removing a food group from the diet may lead to imbalances in vitamins and minerals.

Sometimes, IBS is transient and so you may not need carry out such a diet for a prolonged period of time.

Too Afraid to Ask About Genital Herpes? | Doctor Explains Causes, Symptoms and Treatment 👩‍⚕️

In today’s episode I breakdown a common skin infection - Genital Herpes.

Genital herpes is caused by the virus known as Herpes Simplex Virus (HSV), of which there are two types, 1 and 2. HSV is a common Sexually Transmitted Infection, affecting up to 1 in 8 Australians over the age of 25.

Typically HSV 1 tends to affect the mouth area causing cold sores, and HSV 2 favours the genitalia. However due to changes in sexual practice, the two can be reversed.

The first episode of HSV 2 can be proceeded by a prodrome of feeling unwell, having fevers, chills, glandular swelling and general fatigue. Thereafter there may be some blisters and ulceration which can be sore, itchy and painful.

Usually patients will present to the doctor for treatment. This will usually encompass an examination, including a swab and medication which can take the form of an anaesthetic cream or anti-viral tablets. These will help to reduce the symptom duration. Usually the first episode will last up to 3 weeks.

HSV is transmitted through close skin to skin contact and so it is vital if you are affected not to engage in this practice until symptoms have resolved on the advice of your doctor. Transmission can also occur in asymptomatic people and so the best way to help prevent infection is to use barrier methods.

HSV remains dormant in the body after initial infection and can reoccur especially if your immune system is low or you are stressed. Typically recurrent episodes tend to be less severe and shorter in duration.

It is therefore important to lead an active and healthy lifestyle to reduce your chances of getting a recurrent outbreak.

How Sugar Affects Your Health | Doctor Explains 👩‍⚕️🍭

In today’s episode of Dr Nora, I give you the low down on sugar from how much if any, we should be consuming to how it impacts our health.

RECOMMENDATIONS

Whilst there are no strict recommendations in Australia for the amount of sugar that should be consumed, it is widely agreed that you should not consume more that 10% of your total calorie intake as sugar. For an average woman, this would be 6 teaspoons per day and for an average man, 9 teaspoons.

SUGAR

Sugar belongs to a food group known as carbohydrates. These carbohydrates can either be digestible or indigestible. Indigestible carbohydrates pass through our body unchanged and are known as fibre or roughage. Digestible carbohydrates are sugars and starch.

There are different types of sugars, such as glucose, fructose, galactose, maltose, lactase and sucrose - which is the chemical name of sugar.

When we eat carbohydrates including sugar, our body converts this into glucose which gives us energy. Glucose is maintained in the blood stream by a chemical known as insulin that keeps the sugar level within range. If we do not use the glucose in our blood stream, it gets converted into glycogen which can then be stored for use later.

Glycogen if not used, is converted into fatty acids, which, if there is an abundance of circulates in the body and gets stored as fat for example in the liver. This can lead to a condition known as non-alcoholic fatty liver disease. These changes can often be reversible with a change in lifestyle and diet.

For further studies and trials on the effects of sugar, be sure to read ‘Pure White & Deadly’ by John Rudkin.

Within this book there are a number of studies that demonstrate the adverse effects of sugar on the body, for example, a rise in Triglycerides (a fat within the body), a decrease in our good cholesterol and a stickiness to our blood cells. These changes can contribute to adverse effects on the heart. There are of course other factors than sugars that can cause cardiovascular diseases, such as genetics, smoking, alcohol consumption and leading a sedentary lifestyle.

The book also demonstrates an increase in acid reflux in a cohort of patients as well as a reduced ability to maintain a normal sugar level within the blood stream. As we also know an excess of sugar can lead to dental problems.

NUTRITIONAL LABELS

It is important to take a look at nutritional labels when you are next shopping as sugars can be hidden in foods and consumption may be higher than anticipated.

HEALTHY LIFESTYLE

Managing your sugar consumption is just one facet of maintaining a healthy lifestyle. Eating a healthy balanced diet with fruit and vegetables, limiting the amount of alcohol, stopping smoking and carrying out regular exercises are all important. It is important to seek advice from your medical doctor before embarking on any changes to your diet or exercise regime.

Doctor Explains Apple Watch for Health? | Heart, Hearing & Cycle Tracking 👩‍⚕️

HEART RATE

On average the heart pumps blood 60-100 times a minute. The Apple watch monitors the heart function and it does this by using a sensor on the back, for this to work accurately, during setup you need to enter in your height, weight, gender and age. It gives you an alert if your heart rate is too high or too low.

Having a high heart rate - over 100 beats a minute can be a sign of an irregular heart rhythm, otherwise known as an arrhythmia, An example of this is Atrial Fibrillation. This is where the heart pumps quickly in a chaotic manor which can then cause small plaques or clots to break off from vessels causing serious medical conditions such as a stroke or other heart conditions. So with this regard the Apple watch certainly is useful, in fact there has been cases reported of people being correctly diagnosed via the watch for Atrial Fibrillation, prompting them to seek medical attention and potentially saving their lives.

But what about a low heart rate? This is a heart rate under 60 beats per minute, this can be normal in some situations, for example an athlete may have a lower resting heart rate. Similarly some medications or illnesses such as an under-active thyroid can also cause a low heart rate. It is important that you discuss your target heart rate with your medical doctor so that you can accurately use your Apple watch.

People who have an abnormally high or low heart rate may also have symptoms, such as fatigue, dizziness, lethargy, shortness or breath or fainting. Again if you do have these symptoms despite having a normal heart rate it is important to discuss this with your medical doctor and remember that the Apple Watch doesn’t check for all types of abnormal heart conditions.

ECG/EKG

You may have seen the ECG/ EKG function (electrocardiogram) on TV shows like Scrubs, House, Casualty or Holby City, where we put stickers onto your chest to have a look at the electrical activity of the heart. It helps us to determine any rhythm defects, whether you’ve had a recent heart attack or any structural problems related to having a murmur. This feature is yet to be released to Australia.

MENSTRUAL CYCLE

One of the new health features is the ability to monitor and track the regularity of periods and any associated symptoms. This is particularly useful in patients who have underlying medical conditions such as Polycystic Ovarian Syndrome, or for those who are wishing to get pregnant or those going through the menopause.

NOISE

Another new feature of the Apple watch is the noise application. Background noise ranges from 65-70 Decibels and doesn’t usually cause damage. Long term or repeated exposure to 85 decibels can cause damage, examples include motorbike or dirt bike.

PEDOMETER

We’ve all heard of step counts, step count this, step count that, 10,000 steps. We know that by having an active lifestyle, we’ll be healthier.

However, if you’re doing 10,000 steps a day and you’re eating KFC, Burger King, Bob’s Burgers, you’re probably not on the right track to having a healthy lifestyle.

FALLS DETECTION

The Apple Watch has a gyroscope and an accelerator and when a hard fall is detected an alert is sent to your watch. You can then press on the screen to say you’re ok, or if you remain immobile and don’t disable the alert after 60 seconds it will call the emergency services. This is particularly useful for certain groups of people, such as epileptics or the elderly.

CONCLUSION

As you can see the Apple watch has a number of medical features which have a number of benefits if used correctly. Please note however, that the Apple watch nor the contents of this video are not intended to be a substitute for professional medical advice and should not be relied on as health or personal advice. Always seek the guidance of your doctor or other qualified health professional with any questions you may have regarding your health or medical condition.

Thanks for watching and if you have any questions please don’t hesitate to drop me a line in the comments section below.
